Who Should Consider Vestibular Assessment?
Vestibular assessment may be recommended for patients experiencing:
- Dizziness
- Vertigo
- Spinning sensations
- Imbalance
- Unsteadiness
- Falls or near falls
- Motion sensitivity
- Recurrent positional vertigo
- Ongoing balance symptoms
- Persistent dizziness despite normal imaging
- Suspected inner ear balance disorder
Vestibular testing provides objective information about how the balance organs, eye movement pathways and vestibular reflexes are functioning.
Conditions Commonly Assessed
Vestibular assessment may assist in the investigation of conditions such as:
- Benign Paroxysmal Positional Vertigo (BPPV)
- Vestibular neuritis
- Labyrinthitis
- Vestibular migraine
- Ménière’s disease
- Superior Canal Dehiscence Syndrome (SCDS)
- Bilateral vestibular hypofunction
- Persistent Postural-Perceptual Dizziness (PPPD)
- Chronic vestibular dysfunction
- Unexplained dizziness and imbalance
What Tests Are Included?
The exact test battery may vary depending on the patient’s symptoms, history and clinical presentation.
VNG / Oculomotor Testing
VNG and oculomotor assessment evaluates eye movements and vestibular-related visual pathways.
This may include:
- Spontaneous nystagmus testing
- Gaze testing
- Random saccades
- Smooth pursuit
These tests help assess eye movement control and may provide information about peripheral or central vestibular involvement.
Positional Vertigo Testing
Positional testing is used to assess vertigo triggered by changes in head position.
This may include:
- Dix-Hallpike manoeuvre
- Lateral head roll test
These tests are commonly used in the assessment of BPPV, one of the most common causes of vertigo.
Video Head Impulse Test
Video Head Impulse Testing, also known as vHIT, assesses the vestibulo-ocular reflex.
The vestibulo-ocular reflex helps keep vision stable when the head moves. vHIT provides information about semicircular canal function and can assess all six semicircular canals.
vHIT may assist in the assessment of:
- Vestibular neuritis
- Unilateral vestibular hypofunction
- Bilateral vestibular hypofunction
- Chronic vestibular disorders
cVEMP and oVEMP Testing
Vestibular Evoked Myogenic Potential testing assesses otolith pathway function.
cVEMP assesses saccular function and the inferior vestibular nerve pathway.
oVEMP assesses utricular function and the superior vestibular nerve pathway.
VEMP testing may assist in the investigation of otolith dysfunction and conditions such as Superior Canal Dehiscence Syndrome.
Caloric Vestibular Assessment
Caloric testing is available when clinically indicated.
This test provides low-frequency assessment of horizontal semicircular canal function and may assist in cases where additional vestibular information is required beyond vHIT findings.
Caloric testing may be considered in the investigation of:
- Vestibular neuritis
- Unilateral vestibular hypofunction
- Ménière’s disease
- Chronic vestibular disorders
- Complex or unclear vestibular presentations

What Does the Report Include?
Following assessment, a comprehensive report is provided to the referring practitioner.
The report may include:
- Relevant clinical history
- Objective vestibular test findings
- Interpretation of results
- Clinical impressions
- Recommendations for management or referral where appropriate
Vestibular reports are overseen and signed by a consulting neurologist.
